The MyPD patient mobile application provides peritoneal dialysis clinics and their patients with an interface to collect and view patient therapy exchange and vitals data. If your dialysis clinic is a participating clinic, then your clinic will be able to register you for MyPD and you will be able to use the app to perform the following key functionalities:
. Protect user information by secure account login
. Capture vitals data through Bluetooth as well as manual data entry
. Capture Photos and share with care team
. Capture and view CAPD exchange data
. View APD exchange data
. Automatically share details collected with your care team
. View therapy trends and treatment history
. View prescription details as prescribed by your care team
. Get notified when there is a change to your prescription

Please note you will only be able to register for this app if your clinic is a participating clinic and under the supervision of your healthcare provider.
